INSURANCE

I am able to bill your insurance company on your behalf for sessions as an in-network provider or out-of-network provider. Currently, I am an in-network provider for Regence Blue Cross Blue Shield (and most affiliated Blue Cross plans), Moda, Kaiser Permanente, Aetna and Cigna.  All other plans are billed as an out-of-network provider. I am unable to able to bill Medicaid (OHP) or Medicare at this time. Sliding scale options are available sometimes if clients do not have health insurance or have finacial concerns.

As an in-network provider, a copay or coinsurance amount is usually collected at each session. The amount is dependent on your plan, but is usually a set amount or percentage. In some cases you may have a deductible to meet (see below).

As an out-of-network provider, the full fee (see below amounts) is collected at each session and your insurance is billed for you. After I receive the first insurance payment, we can adjust your portion for each session based on what is paid and refund or credit the difference paid for future sessions.

It is important to know that some high deductible plans require you to meet a deductible first regardless of whether you see a preferred provider or not. In this case, we will collect the full contracted rate for your session.

 

Please contact me or I highly recommend calling your insurance provider if you have any questions about using your insurance to pay for sessions. The number to call is usually on the back of your insurance card.

Questions for your insurance provider:

• Is this a primary or secondary plan? *sometimes people are "double covered" and have 2 insurance plans/providers

• Do I currently have coverage for outpatient mental health services?

• Is Lauren Metzger covered under my plan as an in-network provider or an out-of-network provider?

• What portion of my counseling costs will be covered under my plan and what portion will be my responsibility?

• What is my annual deductible? How much of my deductible have I met for my plan year? Is my deductible waived or do I have to meet deductible before my plan pays for counseling?

• What is my coinsurance or copay if Lauren Metzger is an in-network provider on my plan?

 

Fees 

Initial Intake Session (60-75 minutes): $185

Individual Session (55-60 minutes): $165

Family Session with or without client (60 minutes): $165

* These rates may not apply if you have Moda, Aetna, Cigna, BCBS, or Kaiser insurance based on your plan benefits

Payment is requested at time of service. I accept checks and all major credit cards and HSA/ FSA debit cards.

In-network and out-of-network fees vary depending on your insurance.
